پژوهشگران نوروساینس دانشگاه کالیفرنیا در سانفرانسیسکو 🇺🇸 در پژوهشی کم نظیر دریافتند یک داروی #آنتی_هیستامین معمولی (که از داروخانه بدون نسخه قابل دریافت است) می تواند منجر به #بازسازی مجدد #میلین در #اعصاب تخریب شدۀ #بیماران مبتلا به MS شود.
🔬این پژوهش کاملاً آزمایشی بر 50 داوطلب دارای MS به مدت 150 روز انجام شد. افراد شرکت کننده در فاز #عود MS بوده و #نوروپاتی #مزمن میلین زدایی #عصب #بینایی داشتند و مدت تشخیص زیر 15 سال بود.
📚ارزیابی های #QEEG بر #تاخیر #موج #P100 نشان داد مصرف 90 روزۀ دوز 5.36 میلی گرم #دهانی روزی دوبار #کلماستین #فومارات منجر به بازسازی مجدد میلین در افراد شرکت کننده شده است.
🔅تنها عارضه این دارو #خستگی و #رخوت شرکت کنندگان بود که عارضۀ شایع تمامی داروهای #آنتی #هیستامینی و کاملاً طبیعی است. بر اساس تحلیل های صورت گرفته، حتی برای افرادی با سابقه بسیار بیشتر MS نیز این درمان قابل انجام است.
🔆این پژوهش گام مهمی در درمان (نه تسکین) بیماری #مالتیپل #اسکلروسیس (MS) به شمار می رود و با توجه به نتایج فوق العاده، دسترسی آسان به دارو و نیز ارزان بودن آن می تواند گام مهمی در درمان این بیماری محسوب شود.
Summary
Method: We did this #single-centre, 150-day, #double-blind, #randomised, #placebo-controlled, #crossover #trial (#ReBUILD) in #patients with #relapsing #multiple #sclerosis with #chronic #demyelinating #optic #neuropathy on stable #immunomodulatory #therapy. Patients who fulfilled #international #panel #criteria for #diagnosis with disease duration of less than 15 years were eligible. Patients were randomly assigned (1:1) via block randomisation using a random number generator to receive either #clemastine #fumarate (5·36 mg orally twice daily) for 90 days followed by #placebo for 60 days (group 1), or placebo for 90 days followed by clemastine fumarate (5·36 mg orally twice daily) for 60 days (group 2). The #primary #outcome was #shortening of #P100 #latency #delay on full-field, pattern-reversal, #visual-evoked #potentials. We analysed by intention to treat. The trial is registered with #ClinicalTrials.gov, number NCT02040298.
Findings: Between Jan 1, 2014, and April 11, 2015, we randomly assigned 50 patients to group 1 (n=25) or group 2 (n=25). All patients completed the study. The primary efficacy endpoint was met with clemastine fumarate treatment, which reduced the latency delay by 1·7 ms/eye (95% CI 0·5–2·9; p=0·0048) when analysing the trial as a crossover. Clemastine fumarate treatment was associated with fatigue, but no serious adverse events were reported.
Interpretation: To our knowledge, this is the first randomised controlled trial to document efficacy of a remyelinating drug for the treatment of chronic demyelinating injury in multiple sclerosis. Our findings suggest that myelin repair can be achieved even following #prolonged #damage.

Abstract
Multiple sclerosis (#MS) is the most common #demyelinating disease of the #central #nervous #system. It is classified as being an autoimmune response in the #genetically susceptible individual to a persistent but unidentified #antigen(s). Both the adaptive and the innate immune systems are likely to contribute significantly to MS #pathogenesis. This review summarizes current understanding of the characteristics of MS #autoimmunity in the initiation and progression of the disease. In particular we find it timely to classify the autoimmune responses by focusing on the #immunogenic features of #myelin-derived #lipids in MS including #molecular #mimicry; on alterations of #bioactive #sphingolipids mediators in MS; and on functional roles for regulatory effector #cells, including #innate #lymphocyte populations, like the invariant #NKT (#iNKT) cells which bridge #adaptive and innate immune systems. Recent progress in identifying the nature of sphingolipids recognition for iNKT cells in immunity and the functional consequences of the #lipid-CD1d interaction opens new avenues of access to the #pathogenesis of #demyelination in MS as well as design of lipid #antigen-specific therapeutics.

Multiple sclerosis (#MS) is the most common #demyelinating disease of the #central #nervous #system.
