✳️یک مرد ۳۷ ساله با نژاد قفقازی که سه ماه پیش #سکته_قلبی ایسکمیک داشته است به مرکز درمانی مراجعه میکند :

🔹او قبل از #سکته هیچ دارویی مصرف نمیکرده است .
🔸در روز بستری LDL-C او ۱۴۰ میلیگرم در دسی لیتر بوده است .
🔹در روز ترخیص بیمار نسخه ای با رزوواستاتین ۴۰ میلیگرم و آسپیرین ۸۱ میلیگرم دریافت کرد .
🔸مرور #آزمایش او این مقادیر را نشان میدهد:

▪️LDL-C :84 mig/dL.
▫️Total cholesterol:154 mg/dL,
▪️HDL-C :42 mg/Dl
▫️Triglycerides:140 mg/dL.

✅ #بیمار اذعان داشته است که در طول این مدت بیشترین تغییرات مثبت در نحوه ی زندگی را داشته و به درمان دارویی پایبند بوده است .

📒🗞 با توجه به مطالعه در مجله کالج #کاردیولوژی امریکا که در 22 ژانویه منتشر شده است ، در حالی که حفظ زندگی بیماران #ایست_قلبی در بيمارستان ها بهبود یافته است، بيمارانی که در طول شب و یا تعطیلات آخر هفته #ایست_قلبی داشته اند در مقایسه با بیمارانی که طول روز دچاراین حمله شده اند شانس ادامه حیات کمتری داشته اند .

▫️دکتر Uchenna Ofoma و همکارانش، 151,071 بیمار حداقل 18 سال که تجربه ایست قلبی در بیمارستان را بین ژانویه ۲۰۰۰ و دسامبر سال 2014 داشته اند با استفاده از رجیستری GWTG شناسایی کردند . بیش از نیمی از بیماران قلبی در ساعات off حمله را تجربه کره اند . به طور کلی، 62.4 درصد با تلاش برای احیا جان سالم به در بردند و 18.6 درصد از بيمارستان ترخيص شدند . میزان کوموربیدیتی، محل ، زمان و تعداد عمل مداخله ای در زمان های on و off شیفت یکسان بوده است .

▪️به گفته محققان، درصد پایین تر حفظ جان #بیمار در ساعت شب و تعطیلات به احتمال زیاد به چند عمل وابسته است : تغییرات الگوی نیروی انسانی بیمارستان در طول شب و تعطیلات آخر هفته . #پزشکانی که در شب و تعطیلات آخر هفته فعالیت میکنند ممکن است کمتر به ارائه پوشش درمانی مناسب به بیماران آشنا باشند .نسبت پرستار به بيمار در تعطیلات پایین تر است. تاثیر منفی کار شیفت به خصوص در طول شب، در مهارت های روانی و فعالیت های مهارتی مانند احياء در مطالعات نشان داده شده است.

📌COACT: Coronary Angiography After Cardiac Arrest Without ST-Segment Elevation

• The COACT trial (Coronary Angiography after Cardiac Arrest) is the first published randomized clinical trial evaluating the strategy of immediate coronary angiography versus delayed coronary angiography in patients successfully resuscitated after out-of-hospital cardiac arrest with an initial shockable rhythm and no ST-segment elevation on their electrocardiogram.

• A total of 552 patients was randomized. The primary endpoint was survival at 90 days, at which time 64.5% (176/273) of patients receiving immediate angiography and 67.2% (178/265) of patients in the delayed angiography group were alive (odds ratio 0.89; 95% confidence interval [CI], 0.62-1.27; p = 0.51).

• This is the first report of 9 separate randomized clinical trials examining this issue in post-arrest patients without ST-segment elevation. Over the next decade when all of these trials are completed, more than 4,000 patients will have been studied. The COACT trial was well-done.
